INFORMATION FOR RECORDS APPOINTMENT

 

  The ORTHODONTIC RECORDS APPOINTMENT involves recording the following  information from the patient:  

  • Postural and Cranial Assessment
  • TMJ Assessment
  • Impressions for Dental Study Casts
  • Intra-oral Photographs
  • Extra-oral Photographs
  • Referral for radiographs

 

            It is important that appropriate clothing is worn for this appointment so that the photographs will display the patientÕs structure and support the DoctorÕs visual assessment. Please  observe the following guidelines for the patient when attending the Orthodontic Records Appointment, the Final Records Appointment or as requested by the Doctor for any progress photos during treatment.

PREFERRED ATTIRE FOR ORTHODONTIC RECORDS

  • Swimming Costumes (NOT boardshorts)
  • Rash Shirt / Bicycle pants or shorts
  • Aerobic / Gym style clothes
  • Any close fitting clothes that display body outline
  • NO baggy or oversized clothes
  • HAIR styled to expose ears for face photos
